Tourist	One visa form duly fill & sign by applicant 2 Photos ( recent, passport size photo with white background without border) Covering letter Return Ticket Bank statements last 3 months (Original and photocopy of the passbook with updated entries/print out) including address and telephone no. of the bank Personal ITR last 3 years   Health Insurance  photocopy Tour itinerary and proof of Hotel confirmation along with confirmed hotel booking, address, telephone no. and email address of hotel. Photocopy of passport 1st two and last two pages , air ticket.
